MEDICATION: Our Board of Education policy and the
N.J. Nurse Practice Act specify that if a child needs prescription or
non-prescription medication in school, the parent or guardian must
provide a written doctor's note with the child's name, dose, reason and
time of administration. Forms are available in the nurse's office. The
medication must be in the pharmacy container, and must be brought in by
the parent. Parents may come in and administer medication to their
children at any time.
PHYSICAL EXAMINATIONS: Physical examinations are
required on all kindergarten and new students. Forms can be obtained in
the Health Office. Our school physician is also available for physical
examinations for students who do not have a medical home.
